New Patients

Welcome to Grand Traverse Women’s Clinic. We are excited to be able to serve your medical needs. In order to be successful with your first visit to us we ask that you bring your completed forms with you. (You can download the forms below.) You must also bring your insurance cards AND your driver’s license with you to your appointment as we need to scan these items into our electronic medical chart for completeness.

We ask that you arrive 15 minutes earlier than your scheduled appointment time. This provides adequate time to enter your information into our system and ensures that you are on time for your provider. Please note that we ask for 24-hour notification if you are unable to make your appointment. If there are frequent no-call, no-shows we reserve the right to discharge patients from the practice. Please note; if you arrive 10 minutes or more after your scheduled appointment time, you may be asked to reschedule.

We do ask that you only bring one adult support person with you to your appointment.  If you need to bring children with you to your visit we ask that you keep them contained either in a stroller, a car seat or on your lap if appropriate, while in the exam room.

Please note; we will collect your copay at time of check-in. It is most helpful if you know your copay amount.

Parking
Turn right onto Brook St. off of 6th St., then turn right, directly into our parking lot on the west side of the building. This lot serves Grand Traverse Women’s Clinic and Traverse Heart and Vascular. There is additional parking across the road or around the building at one of the metered parking spots.
